Christmas Parade Band Application

Cypress Pointe Hospital is proud to present the Rotary Club of Hammond's 60th Annual Christmas Parade, Saturday, December 1, 2018, starting at 5:30pm.
This year's theme: "A Louisiana Christmas"

You can use the below form to submit your band application.







SCRIPT (50 words or less), INSURANCE, INDEMNIFICATION and DRIVER’S LICENSE are due with application. REMEMBER we need a copy of vehicle insurance and drivers license if you have a vehicle participating or pulling a float. No vehicle will be allowed in the parade without proof of insurance and driver’s license. We also need a completed Indemnification Agreement signed by the director/in charge person. Payment, insurance, Indemnification Agreement and driver’s license must accompany application.

Lineup will be determined by the return date of application.



If you do not wish to submit online, your Script, Application, Indemnification Agreements, Proof of Insurance and Driver's License may be mailed to Hammond Rotary Club, Inc., P.O. Box 1486, Hammond, LA 70404, or emailed to parade@hammondrotary.com.
